Family Support Services Information Packet & Application

 
The new year for the Family Support Services (FSS) program began Jan 1. To be considered for funds from this program, each year you must complete the enclosed eligibility form and return it.

Please note that the income you report should be your taxable income as reported on your most recent Federal Income Tax form. For families who have adult children using this program, the income should be the family income, not just the individual's income. You are eligible to apply for this program because you are a family unit and the individual continues to live at home. For the current calendar year you must verify your most recent taxable income.

Included with the Application & Eligibility Determination form are: a pamphlet briefly describing the FSS program, a Funding Request Form, and a Mileage Reimbursement form. Please remember, except for emergency situations, these requests must be submitted for approval prior to a service being rendered. Requests submitted without prior approval will not be eligible for funding. Mileage does not require prior approval, but must be submitted within 30 days of the first date recorded on the form. All approved requests should be purchased and submitted for reimbursement within 30 days of approval.

Once we have received your completed packet (Eligibility Form, tax return), you will be notified of funds available to your family and whether or not a co-pay is required. Please note, if your child's file lacks required information (copy of birth certificate, social security number, proof of eligibility or current plan), you will not be approved for this program until all items are current in the file.
